I will bring with me the paperwork and a care plan we will personalize together.

STEP 1

PAYMENTS AND INSURANCE

If you have insurance, fill out The Lactation Network information to see if you’re covered.

Payment is due at the time services are provided. 

I accept HSA, Credit Card, Zelle, or Venmo. 

If you are not covered through Lactation Network or Wildflower, most insurances still cover lactation, but you would privately pay and then submit the super-bill I will provide you. Reimbursement from your insurance company is not guaranteed.

STEP 2

Health SAFETY and Our visit

1. Please wash your hands upon my arrival. I will also ask to wash my hands upon arrival. I will wear gloves when appropriate.

2. Please reschedule if you or someone you have been around has Covid or Flu symptoms, a cold or fever.

3. I will be using a medical-grade sanitizing wipe on all surfaces between each client.

4. We will create a careplan together. You do not need to take notes or remember anything. It will be in writing.

STEP 3

GENERAL INFORMATION

RESCHEDULING AND CANCELLATIONS

I often schedule clients back to back, so being on time is so appreciated. If you are more than 15 minutes late, we may need to reschedule. If you are a no-show, you will be charged $50. Please give at least 12-hour notice to change or cancel appointments.

HOME VISITS and PETS

For those of you who I am coming to your home, I ask that animals be put in another room or outside.  I have a sensitivity to animal hair, and I find it can cause distractions during the visit.  Thanks so much for understanding!
